Google Forms Quiz to Certificate: Only If They Pass

A certificate should mean someone passed. This guide shows you how to issue certificates automatically from a Google Forms quiz, but only to learners who answer correctly. You'll use two forms: a hidden certificate claim form connected to CertFusion, and a quiz form that uses answer-based branching to reveal the claim link only on the correct-answer path.

Why two forms?

Google Forms has no built-in way to say "issue a certificate if the score is above X". What it does have is section branching — the ability to send someone to a different section depending on which answer they picked.

So you split the job in two:

• Form 1 — the certificate claim form. Name and email only. CertFusion is connected to it, so every submission issues a certificate. This form is never shared publicly.
• Form 2 — the quiz. Learners take this one. A wrong answer lands on a "try again" section. A correct answer lands on a section that contains the claim form link.

Because the claim form link only appears on the correct-answer path, people who fail never see it.

Prerequisites

• A CertFusion account with at least one certificate design ready
• The CertFusion add-on installed in Google Forms
• A Google account with access to Google Forms

Step 1 — Build the certificate claim form

Create a blank Google Form and name it something like Certificate Claim Form. Add exactly two short-answer questions and mark both as required:

• Your Name
• Your Email Address

These are the only two fields CertFusion needs to issue and deliver a certificate.

Step 2 — Open the CertFusion add-on

With the claim form open, click the add-ons (puzzle piece) icon in the top toolbar, choose CertFusion, then Open CertFusion.

Step 3 — Pick a template and map your fields

The CertFusion panel opens on the right, already signed in to your account. Choose the certificate template you want this form to issue, then map the two fields:

• Email Field → Your Email Address
• Name Field → Your Name

If your template has custom fields — a course name, a date, a CPD point value — you can map extra form questions to them here.

Step 4 — Turn on Auto Send and save

Leave Auto Send enabled so certificates go out as soon as someone submits, then click Save Configuration. You'll see a confirmation that CertFusion is now active on this form.

Turn Auto Send off instead if you'd rather collect recipients in CertFusion and issue certificates in bulk later.

Step 5 — Publish the claim form and copy its link

Click Publish, then the link icon to open Copy responder link. Tick Shorten URL and copy it. Keep this link somewhere handy — it's the piece that connects your quiz to your certificate.

Do not share this link with learners directly. Anyone who has it can claim a certificate.

Step 6 — Create the quiz form

Create a second blank form. This is the one your learners actually receive. Add your question and your answer options — in the demo there's one wrong answer and one correct answer, but in a real quiz you'd use your own course questions.

Step 7 — Add the wrong answer section

Add a second section. Name it something like Wrong answer section and add a short description such as "Thank you and please try again!". This is where failing learners land — no claim form, no certificate, and they can go back and retake the quiz.

Step 8 — Add the correct answer section with the claim link

Add a third section, name it Correct answer section, and paste your claim form link into the description. This is the reward path.

Google Forms adds an empty question to each new section — delete those, you don't need them. A section with only a title and description works fine.

Step 9 — Turn on answer-based branching

Go back to your quiz question, open its three-dot menu, and choose Go to section based on answer.

Each answer option now gets its own destination dropdown. Set them up like this:

• Wrong answer → Go to section 2 (Wrong answer section)
• Correct answer → Go to section 3 (Correct answer section)

That single setting is the entire pass-or-fail rule. If your quiz has several questions, put the branching on the final question, or branch each question into its own follow-up section.

Step 10 — Publish the quiz and share its link

Publish the quiz and copy its responder link. This is the link that goes out to learners — never the claim form link.

Step 11 — Test both paths

Open the quiz as a learner would. Pick the wrong answer and continue: you land on the try-again section with no way through.

Now clear the form and answer correctly. The correct answer section appears with the claim form link.

Open the link, fill in the name and email, and submit.

Step 12 — Confirm the certificate was issued

Back in CertFusion, open your certificate design and go to Recipients. The learner appears immediately, and their certificate moves through Processing to Issued and gets emailed automatically.

Frequently asked questions

Can I require more than one correct answer before issuing a certificate?

Yes. Chain the branching: each question's correct answer continues to the next question's section, and any wrong answer jumps to the fail section. Only a learner who answers every question correctly reaches the final section with the claim link.

Can someone skip the quiz and go straight to the claim form?

Only if they have the claim form URL. Keep it out of your course materials, emails and site navigation, and it exists solely inside the correct-answer section. For an extra layer, restrict the claim form to signed-in users in one organisation via Google Forms settings.

Does this work with Google Forms' built-in Quizzes mode and a score threshold?

Google Forms' Quizzes mode grades and shows scores, but it can't branch on a total score — only on individual answers. Answer-based branching, as used here, is what actually gates the claim form.

Will the learner's name on the certificate match their quiz answers?

The certificate uses whatever they type into the claim form. If you need it to match a roster, collect the name on the quiz form too and check the responses, or import your recipients into CertFusion in advance.

Does the certificate send automatically?

Yes, as long as Auto Send is on in the CertFusion panel. Turn it off if you'd prefer to review recipients and issue in bulk.
